Forms for New Students

OCEAA will contact you when a space becomes available for your child.

At that point you will need to provide:

1. Child’s Certified Birth Certificate
2. Proof of Current Immunizations
3. Physical and Dental exams
   • Report of Health Examination Form
   • Form Oral Health Assessment 
4. Copies ELPAC and CAASPP scores (if applicable)
5. IEP/504 (if applicable)
